¹ QTS requires an ORIGINAL copy of a medical order for self-administered oxygen signed by the individuals medical practitioner to provide Medical Oxygen.

Ambulatory - Rider is able to ride without the use of a wheelchair or stretcher.
Wheelchair - Rider requires a wheelchair during transportation.
Stretcher - Rider requires a stretcher during transportation.
Electric/Double Wheelchair - Rider Requires wheelchair and stairs or limited access or electric mobility equipment or passenger 225+ pounds.
Double Stretcher - Rider requires stretcher and stairs or limited access or passenger 225+ pounds.
